The plain label, and overwhelmingly about fiction: あのキャラはヤンデレだ. Said of a real person it is an accusation, not a tease.

病む+デレ(デレ)

Both halves live outside the compound: 病んでる is ordinary slang for being unwell, デレデレ for being visibly smitten. The word is a sum.

ツン / ヤン / クー / ダン+デレ

デレ became a suffix. Name the surface manner, attach デレ, and you have described a character with a switch in them — fully productive.

Meaning

ヤンデレ vs ツンデレ — armour vs illness

The first two morae are the whole difference. ツン is from ツンツン, prickly; ヤン is from 病む, to fall ill. A ツンデレ hides warmth behind coldness and the joke is the gap. A ヤンデレ hides nothing — the affection is on display, and it is the affection that has gone wrong.

So the archetypes point in opposite directions. The ツンデレ arc is a thaw: hostility softening into fondness, and the audience waiting for it. The ヤンデレ arc is a descent: sweetness curdling into jealousy, surveillance and worse, and the audience watching it come. One is a romantic comedy device; the other is a horror one.

デレ is a productive suffix now. クーデレ is クール — flat, unreadable, then quietly soft. ダンデレ is 黙り, silence: too shy to speak until they are not. デレデレ on its own is someone with no cold phase at all. Fandom coins more of these than stick, and the pattern is transparent enough that a new one is readable on sight.

It is not a compliment, and it is not dictionary Japanese. The word is internet coinage from 2000s otaku culture, built on the back of ツンデレ, and it lives in fandom rather than in ordinary speech. Calling a friend ツンデレ is teasing; calling them ヤンデレ says they are dangerous, and the real-world word for that neighbourhood — メンヘラ — is worse.

The kanji 病 is illness: 病気 sickness, 病院 hospital. The verb 病む is literary for the body and current slang for the mind — 病んでる is what a friend says about a bad month. In ヤンデレ it is the love that is ill.

デレ is a sound, not a word

Japanese has a huge stock of mimetics for manner, and デレデレ is an ordinary one: soft, fawning, visibly smitten. Fandom clipped it to デレ and made it a suffix, which is why クーデレ and ダンデレ are readable the first time you meet them.

They are near-opposites. ツンデレ is cold on the outside and warm underneath; ヤンデレ is warm on the outside and broken underneath. English discussion blurs them constantly, and the Japanese keeps them apart in the first two morae.

Calling a real person ヤンデレ

ツンデレ works as affectionate teasing about a real friend. ヤンデレ does not — it says someone's attachment is a danger. If what you mean is clingy, the words are 重い or 束縛が強い.

Confusing it with ヤンギレ

ヤンギレ is 病む + キレる, to snap: violence with no devotion behind it. The デレ is the load-bearing part of ヤンデレ — take it away and the archetype is a different one.

Expecting it in a dictionary or a textbook

It is fandom vocabulary of recent coinage. Ordinary Japanese speakers outside anime circles may know it and will not use it, and no JLPT level lists it — the N1 tag here means you will meet it in the wild, not that it is on any syllabus.
